Assignment of Dental Insurance Benefits
As a service to our patients, we accept the assignment of benefits, which means that your dental insurance provider will pay our dental office directly for the whole or a percentage of the cost covered. We will accept assignments of insurance benefits under the following conditions:

The patient must have a complete new patient examination.

The patient (or person responsible for the account) must pay their deductibles and estimated co-insurance at the end of every appointment.

The patient accepts responsibility for their account. We have no relationship with your insurance company, and you are responsible for any balances on your account after the insurance company has paid us.
